APPLICATION DOCUMENTS FOR REGISTRATION AS A PSYCHOLOGIST
Only current versions of application forms will be accepted.
The current version for this application is version 2006-1. This version of
your application is clearly marked on the first page, and at the bottom of
every page. All other versions will be returned to the applicant.
The following documents are required for application as a psychologist or
psychologist candidate. Some of the larger documents, including the application
form, may be slow to load on your screen. The screen may appear blank for a up to 30 seconds, and then the document will appear.

PLEASE BE AWARE THAT ALL PRIVATE PRACTICE-BASED SUPERVISORY ARRANGEMENTS MUST RECEIVE PRIOR PAM EXECUTIVE COUNCIL APPROVAL.
The following documents will be required later for most applicants, when you are ready to take the examinations. Do not print them until you are ready to use them, as they may be revised from time to time.
