APPLICATION DOCUMENTS FOR RECIPROCAL APPLICATIONS



This application package is for use only by applicants who are already licensed or registered in another jurisdiction in Canada or the United States, and who are eligible for Reciprocal Registration. Please review the categories of eligibility available on the main page of this website. Follow the link "Registation by Reciprocity Information" to determine if you are eligible.

Only current versions of application forms will be accepted.

The current version for this application is version 2006-1. This version of your application is clearly marked on the first page, and at the bottom of every page. All other versions will be returned to the applicant.

You need to print all of the required documents, and fill them out. These documents cannot be filled out on-line.

The following documents are required for a reciprocal application as a psychologist or psychological associate. Some of the larger documents, including the application form, may be slow to load on your screen. The screen may appear blank for up to 30 seconds, and then the document will appear.

Note that reciprocity applicants are not required to submit a separate application for an oral interview. Your reciprocity application is considered an application for oral interview. If for some reason you are not eligible for reciprocal registration, your oral interview fee of $300 (included in the application fee) will be refunded. The application processing fee of $200 is non-refundable.
